South Downs Leisure Exercise Referral Scheme
South Downs Leisure are pleased to announce they are launching their brand-new Exercise Referral Scheme and patients can be referred immediately. Their new Exercise Referral Consultants - Emma (Worthing sites) and Jo (Adur sites), two highly experienced and qualified exercise specialists, will be managing the scheme.
Overview
- Clients can be referred by GPs, medical professionals and Social Prescribers (Going Local).
- The referral scheme lasts for 12 weeks - NEW cost of £40.95 a month (paid upfront for each month or in 3 direct debit instalments).
- The scheme is client centred and the Exercise Referral Consultant will work with individuals to design a programme to suit their needs and goals.
- Clients have the opportunity to use the gym, attend classes and swimming is also included (clients can attend as many sessions as they like in these 12 weeks).
- Clients must be 18+ years of age.

Process
- An online referral form must be completed and submitted.
- The online application form can be found on the South Downs Leisure website.
- The Exercise Referral Consultants will then contact the client.
- There will be an initial one-to-one consultation and induction with a Clinical Exercise Specialist, which may take up to 90 minutes.
- This will be followed up in week 2 and 6 with a programme review to ensure the suitability of the programme and progressed as required.
- The scheme is completed with an exit interview where progress and future options will be discussed.
- Once the 12-week exercise referral scheme has finished, clients can join FIT4 with no joining fee (saving of £49.50) and get a reduced monthly rate of £40.95 (compared to regular £49.95).
Exclusions
Clients will not be eligible to join the scheme if they have any of the following contraindications to physical activity:
- Unstable Angina
- Uncontrolled Diabetes
- Unstable mental health
- Uncontrolled Arrythmia
- Recent acute soft tissue injury
- Systolic Blood Pressure 180mm/Hg at rest
- Diastolic Blood Pressure 100mm/Hg at rest
- Uncontrolled Tachycardia 100bpm at rest
- Unstable or acute heart failure
Inclusions
Clients are eligible to join the scheme if they are inactive and have any of the following health conditions:
- Osteoporosis
- Controlled Diabetes
- Stroke
- Osteoarthritis/ Rheumatoid Arthritis
- Cancer
- Mild to Moderate Mental Health Condition
- Heart Disease
- Respiratory conditions (including Long Covid)
- Controlled Hypertension
- Unhealthy Weight (BMI>28)
- High Cholesterol Levels
- Musculoskeletal Rehabilitation
- Back Pain
- Joint replacement
- Falls prevention
